Job Title: Case Manager

FLSA Status: Full Time, Exempt

Department: Catholic Charities Community Services of Orange County: Housing Resource Center

Reports to: Senior Case Manager

Supervisory Responsibilities: No 

Summary: This position provides direct case management services to the needy and disenfranchised through advocacy, information and referral, crisis intervention services, and short-term case management. The position will also have a role in the reception area answering telephones and processing consumers entering the HRC for information and or services.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

1. Provides crisis intervention to homeless individuals and families facing housing emergencies.
2. Conducts thorough needs assessment and develops a service plan to address identified, concrete needs using a short-term intervention model.
3. Maintains current and accurate documentation of services provided to clients.
4. Maintains government grants by obtaining supporting eligibility documentation.
5. Provides short-term case management focusing on assessing factors precipitating housing crisis.
6. Develops and implements preventive measures to reduce risk of eviction in the immediate future.
7. Places homeless consumers in housing.
8. Maintains HMIS Database for consumers served through HRC.
9. Answers telephones and provides information needed.
10. Develops and maintains working relationships with landlords in the Newburgh Area and surrounding communities.
11. Participates in regular supervisory sessions with immediate supervisor.
12. Attends Agency's in-service training.
13. Completes daily, weekly, and monthly statistical reports as required.
14. Develops and maintains good communication with other programs and departments within the organization to facilitate inter-agency referrals.
15. Interacts with Department of Social Services Staff and prepares needed documentation for cases and shared cases.

Position Type and Expected Hours of Work:

This is a full-time position. Days and hours of work are generally 9:00 AM to 5 PM Monday - Friday. Additional hours may be required to meet program deadlines, or client needs.

  • Exposure to outside weather conditions.
  • Ability to work with the public and/or under potentially stressful conditions.
  • Remain stationary at a work station and use a computer at least 50% of the time.
Qualifications

Education and/or experience required:

  • Bachelor’s degree in Social Work or Social Services field, or Associate’s degree with one year experience in Human Services, or High School Diploma/GED with two years’ experience in Human Services.
  • Must possess a clean NYS Driver’s License, and maintain it clean throughout employment.
Skills, Licenses, and/or competencies required:
  • Knowledge of social service delivery systems and county resources.
  • Knowledge of Housing stock available in Orange County.
  • Computer literate with knowledge of Microsoft Word, Excel, and Outlook.
  • Excellent interpersonal and communication skills.
  • Good organizational, time-management and communication skills.
  • Individual must possess the ability to work well independently as well as part of a team.
